how to add a program to the right-click menu?

Add a shortcut to your program's executable in the SendTo folder. Then right click the file you wish to open in your program,
scroll and select SendTo and choose your program.

In Win7 the SendTo folder is located:
C:\Users\<windows logon profile name>\AppData\Roaming\Microsoft\Windows\SendTo
